2. Message Frequency: Message frequency may vary depending on the type of communication account status.

3. Potential Fees for SMS Message: Please note that standard message and data rates may apply, depending on your carrier’s pricing plan. These fees may vary if the message is sent domestically or internationally.

 

4. Opt-In Method: You may opt in to receive SMS messages from Chosen Life Specialists, LLC in the following way: By submitting an online form

 

5. Opt-Out Method: You can opt out of receiving SMS messages at any time. To do so, simply reply "STOP" to any SMS message you receive. Alternatively, you can contact us directly to request removal from our messaging list.

9. Providing Telephone Numbers and Duty to Notify: You verify that the mobile number provided to Us is true and accurate. You verify that you are the current subscriber or owner of any telephone number that you provide. Should any of your contact information change, including ownership of your telephone numbers, you agree to immediately notify us before the change goes into effect by emailing us at ChosenLifeSpecialists@Gmail.com.

Opt In/Your Consent to Receive Automated Calls/Texts: By opting in or checking the YES box, you agree to receive SMS/MMS messages from Chosen Life Specialists, LLC for 'conversation' purposes at the phone number provided. You may reply STOP to opt out at any time. Reply HELP for assistance. Message and data rates may apply. Message frequency will vary. You acknowledge that by voluntarily providing your telephone number(s) and checking the box, you expressly agree to receive from Chosen Life Specialists, LLC,  concerning clients, and understand that consent is not required to make any purchase from Us. Message and data rates may apply. The Program allows Users to receive SMS/MMS mobile messages by affirmatively opting into the Program, such as through online or application-based enrollment forms. Regardless of the opt-in method you utilized to join the Program, you agree that this Agreement applies to your participation in the Program.

Consent is not a condition of purchase. Msg frequency of 6 per month is average for clients with appointments. Msg & data rates may apply. Mobile Opt-in data will not be shared with third parties. Unsubscribe at any time by replying STOP to opt out; no further messages will be sent. 11. Opt Out Instructions: Your consent to receive automated texts is completely voluntary. You may opt out at any time. To opt out of text messages, reply STOP to any mobile message from Us, or email ChosenLifeSpecialists@Gmail.com and specify that you want to opt out of text messages. You may also text HELP or email us at ChosenLifeSpecialists@Gmail.com.com for help. You acknowledge and agree to accept a final text message confirming your opt-out. If you do not wish to continue participating in the SMS/MMS text message 'conversation' communication or no longer agree to this Agreement, you agree to reply STOP to any mobile message from ChosenLifeSpecialists@Gmail.com to opt out of the Program. You may receive an additional mobile message confirming your decision to opt out. You understand and agree that the foregoing options are the only reasonable methods of opting out.  You also understand and agree that any other method of opting out, including, but not limited to, texting words other than those set forth above or verbally requesting one of our employees to remove you from our list, is not a reasonable means of opting out.

​12. Participation Requirements: By participating in the Service, you acknowledge and agree that you are eighteen (18) years of age or older. You must have a wireless device of your own, capable of two-way messaging, be using a participating wireless carrier, and be a wireless service subscriber with text messaging service. Not all cellular phone providers carry the necessary service to participate. Check your phone capabilities for specific text messaging instructions.

13. Miscellaneous: You warrant and represent to Us that you have all necessary rights, power, and authority to agree to these Terms and perform your obligations hereunder, and nothing contained in this Agreement or in the performance of such obligations will place you in breach of any other contract or obligation. If any provision of this Agreement is found to be unenforceable or invalid, that provision will be limited or eliminated to the minimum extent necessary so that this Agreement will otherwise remain in full force and effect and enforceable. We reserve the right to change this Agreement from time to time. Any updates to this Agreement shall be communicated to you. You acknowledge your responsibility to review this Agreement to be aware of any such changes. By continuing to participate in the Service after any such changes, you accept this Agreement, as modified.

We respect your privacy. We will use information you provide through the Program to transmit your mobile messages and respond to you.

Chosen Life Specialists, LLC does not sell, share, trade, or exchange Personal Information (as those terms are defined in Applicable Law) for itself or any unaffiliated third party. Mobile Opt-in, SMS Consent,  and phone numbers collected for SMS communication purposes will NOT be shared with any third party or affiliates for marketing purposes. Although Chosen Life Specialists, LLC does NOT sell, share, trade, or exchange Personal Information, clients reserve the right to opt out (i.e., put in writing/record) of the sale, sharing, trading, or exchanging of their Personal Information by emailing us at Chosen life Specialists@Gmail.com or submitting a form on the contact us web page (https://www.chosenlifespecialists.com/contact-us).

WE DO NOT SELL, RENT, LOAN, TRADE, LEASE, OR OTHERWISE TRANSFER FOR PROFIT ANY PHONE NUMBERS OR CUSTOMER INFORMATION COLLECTED THROUGH THE PROGRAM TO ANY THIRD PARTY.  Nonetheless, Chosen Life Specialists, LLC, reserves the right at all times to disclose any information as necessary to satisfy any law, regulation, or governmental request, to avoid liability, or to protect Chosen Life Specialists, LLC's rights or property. When you complete forms online or otherwise provide Us information in connection with the Program, you agree to provide accurate, complete, and true information. You agree not to use a false or misleading name or a name that you are not authorized to use. If, in Chosen Life Specialists, LLC's sole discretion, Chosen Life Specialists, LLC believes that any such information is untrue, inaccurate, or incomplete, or you have opted into the Program for an ulterior purpose, Chosen Life Specialists, LLC may refuse you access to the Program and pursue any appropriate legal remedies.
