Chosen Life Specialists, LLC, provides wellness services that primarily support social-emotional health, relationship education, modest dance, and mild recreational fitness connected to coaching plans and goals, etc. In the course of providing Services, Chosen Life Specialists, LLC receives from individuals, itself collects and creates, and processes Personal Information, which may include the following: (1) identifiers, such first and name, date of birth, address, email, and phone number; (2) professional or business information, such as job title, social-emotional information, background, history, and (3) familial data, such as marital status and information about your family members, and any details related to the goal of our spiritual counseling and coaching services.

The last four digits of your SSN may also be collected if you choose to apply for an Online Payments account. Race, ethnic origin, sexual orientation and preferences, and religious or philosophical beliefs are only collected if you choose to enter them into the Services for your Professional Website, as applicable.

3. Social Media

Chosen Life Specialists, LLC maintains and manages business accounts/channels on various social media platforms such as Facebook, Instagram, LinkedIn, X (formerly known as Twitter), Pinterest, YouTube (Social Media), etc. We may collect Personal Information from or about you on our Social Media. Any information, including Personal Information, that you share on Social Media may be available to other members who use the Social Media. Chosen Life Specialists, LLC has no control over the policies and practices of such individuals.
